Step-by-step procedure

- 1Find a general practitioner near you, for example via Doctolib or the Ameli directory. Appointments are often made online.
- 2On the first date, just say that you want to declare him as a treating doctor.
- 3Complete the doctor's statement together: your name, your Social Security number and the doctor's contact information.
- 4The doctor sends the report to the CPAM. This process is free and quick.
- 5If you change your doctor, make a new statement. You can do it when your situation changes.
Common mistakes

- Go straight to a specialist with no orientation. With the exception of care courses, reimbursement falls, except for some direct access.
- Forget to repeat the statement after a change of doctor.
