Step-by-step procedure

  1. 1Identify the basis: professional activity in France or stable and regular residence. Foreign students use the dedicated portal; the other first affiliations usually go through form S1106 and the CPAM.
  2. 2Send only the pieces corresponding to your situation. The CPAM may request additional information, in particular for foreign civil status and regularity of stay.
  3. 3After opening, get the certificate of rights. A provisional number may precede the final identification and the permanent Social Security number.
  4. 4Order the Vitale Card when your folder allows, via the ameli account or the received form. Do not base your actions on a guaranteed period of two or three weeks.
  5. 5Adults have their own rights PUMa and are in principle no longer entitled to another adult; minor children may be attached to a parent.
  6. 6Declare a treating physician to follow the path of care. The Vital Card transmits the refund data, but the paying third party and the absence of advance are not systematic.
  7. 7In case of low resources, check separately the complementary health solidarity (C2S), free or paid according to income.
