Step-by-step procedure

  1. 1Contact the MDPH of the main residence. Check whether the department uses MDPH online, its own portal or a paper repository.
  2. 2Identify needs rather than the only diagnosis: income, human assistance, material, card, education, employment, housing or orientation. Do not check all boxes unrelated to the situation.
  3. 3Write the life project in a concrete way: impossible or difficult acts, time to help, risks and extra costs. Diagnosis alone rarely describes all the consequences.
  4. 4Complete the medical certificate with the functional impact and attach the relevant balance sheets. Check the form version, dates and signatures.
  5. 5Keep a full copy and proof of deposit. Respond to requests for parts and report significant developments.
  6. 6The CDAPH notification responds to each request. Read the reason, duration, paying or executing agency and remedy: notification does not always automatically trigger payment.
  7. 7In case of refusal or partial response, follow the route and time limit indicated. Explanation, conciliation or prior administrative remedy may apply according to law; an association or a lawyer may help.
